The process utilises a range of inspection and laboratory techniques. Although failure analysis generally focuses on the physical cause/s of failure, a thorough investigation is one that looks to identify other potential root causes or contributory factors that led to the failure event, such as human or latent roots.
Establishing the sequence of events that led to the failure is paramount to understanding and establishing the primary cause.
