Failure analysis is an extremely important and complex process required to establish the physical root cause of a failure and to ensure the continued safe and reliable performance of a vast array of engineering structures and components.  

The process utilises a range of inspection and laboratory techniques.  Although failure analysis generally focuses on the physical cause/s of failure, a thorough investigation is one that looks to identify other potential root causes or contributory factors that led to the failure event, such as human or latent roots.

Establishing the sequence of events that led to the failure is paramount to understanding and establishing the primary cause.  Careful attention must be paid to specific details and conditions that the component was exposed to prior to the failure, such as environmental factors, loading conditions and service-life etc.
